Order Confirmation Form (Required)
Completion of the Order Confirmation Form is required after purchase to authorize processing and shipment.
- If the form is not completed in full within 2 business days of purchase, the order will be canceled, refunded as store credit, and subjected to a $200 restocking fee.
- Orders placed using installment payment options such as Klarna or Shop Pay are subject to an additional 15% restocking fee, plus the $200 restocking fee.
